We have created this privacy policy to demonstrate our firm commitment to our member’s privacy and the protection of your information.

The use of Cookies

The my95210 Health Tracker uses cookies to enable members to navigate between pages within the 95210 platform and remain logged in as a member. Since much of the information displayed is according to membership, this is a necessary component of our website. Sharing Information with Third Parties

The privacy of both our visitor's and member's information is very important to us, and, simply put, we do not share any personal information with any party outside one's own work or registered group, including businesses, organizations, schools, sponsors or other third parties, unless a member has provided permission for us to do so.

Display of Personal Information

my95210 Health, Inc. has three specific levels of privacy in each member's Profile "Privacy Management" section that members may choose from to limit the view of their data by other members of their group or by admins of their organization.

A) "I wish to be Anonymous" This Profile Management feature (check box) allows the member to be anonymous by removing their name on the member’s group page and hiding their homepage to all viewers. However, this will not remove their name or data from any group challenges (if the member is participating in a group challenge) to allow for a transparent competition.

B) "Turning off display of individual data" This Profile Management feature (multiple check boxes), allows members to selectively turn off individual data such as steps, fruits and veggies, etc. so that other members of their group or team may not view this particular data. For the monthly Fruits & Veggies and Steps Challenge Leaderboards only, turning off the display of Fruits & Veggies and/or Walking Steps will completely remove the member from the leaderboards.

C) "Remove My Data from Group/Client Reports" This Profile Management feature (check box) will remove the member's name and data completely from the group/client’s reports. The member's data will still be viewable by other members, unless one or more of the privacy management check boxes above are selected. Group admins continue to view the member's name and weekly and monthly point scores online and maintain the ability to remove a member from their group.

The following information is provided in group/client reports (unless the member selects the check box to remove their data from group/client reports): member name, weekly, monthly, and yearly point scores, number of entries, sleep points (not sleep hours), tasks, quiz scores, number of fruits & veggies consumed, exercise minutes, cups of sodas, walking steps, cups of water, tobacco cessation days, and percentage of weight change (not actual weight).

Members may choose to make the following information available to the public such as 1) health journals, 2) gratitude journals, and 3) volunteer journals, by selecting a check box to change the privacy from private to public with each entry.

Individual member information, other than those features mentioned above are private and are not displayed publicly, shared with the organization or other members, or provided to any third parties or sponsors.

Personal information such as address, city, state, postal code, phone number, sleep hours, screen time, custom tracking items (tracking items added by the member), health and gratitude journal entries (not made public), actual weight, blood sugar level, and BMI collected when members register or during daily tracking, are only used to identify members, provide for personal tracking (only the member sees that information), or provide rewards when available, and are never disclosed to their organization (client), other members, third parties, or sponsors.

We will not share any personal information of children 17 years or younger with any party without express permission of the parent.  As of 05/25/2018, we are working on making all data of children 17 years and younger completely private (with privacy management features to turn-on the display of certain data to the child’s group), and automatically including a “parental or guardian email address” field and consent email to parents or guardians to allow the child to register with my95210 or other 95210 group/client.

When registering with my95210, members may use a pseudonym (fictitious or made up name), and also may use a fictitious email address (as long as no other member is using that same email address) to be completely anonymous to other members or to the group/client. If a member chooses to use a fictitious email address, some features of 95210 will not be available, i.e. notifications, Buddy Challenges, as the member will not be able to check the fictitious email account.  This information may also be changed at any time in the member’s Profile.

Disclosure Required by Law

my95210 Health, Inc. may disclose personal information if required to do so by law or in the good-faith belief that such action is necessary to: (1) conform to the edict of the law or comply with legal process served on us or our affiliates, or (2) act under exigent circumstances to protect the safety of the public or members of this website.

Third Party Privacy Policies

my95210 Health, Inc. is not responsible for the privacy practices of third party affiliated sites. We encourage you to read the privacy policies of these sites.

Security

my95210 Health, Inc. implements and maintains reasonable security measures and practices to protect all personal information from unauthorized access, destruction, use, modification, or disclosure. These procedures apply to the collection and storage of personal data, as well as in instances where such data is provided to third-parties mentioned above.

Members who wish to discontinue their membership may delete their account at any time by selecting the Edit Profile link and clicking "Delete my Account" at the bottom of the page. All data associated with the account will be removed and cannot be retrieved.
